We occasionally take photos and videos during events for use on our website, social media, and promotional materials.

Transportation Authorization 

Our church offers transportation to and from church services and events. By signing, I give permission for my child(ren) to ride in the church vans operated by authorized church drivers for scheduled activities.  

Authorization Pickup Contact 

List anyone other than yourself who is authorized to pick up your child(ren). They will need to provide an ID if staff requires.

Student Covenant Agreement
